I say that when I'm mad with someone or something, so "mi girano i coglioni" to do that thing, or to see that persone. It can be used when you are bored

Being very angry about something that recently happened or
being pissed off about something.
It is not a refined expression. You could also say "Mi gira il cazzo", "mi girano le palle" and so on..
