I have found: He's up my ass. (I don't know if the meaning is the same of "Rompe i coglioni" because there is the word ass)
"Rompe i coglioni" = (literally) he breaks my nuts.
Comunque è volgare ma le persone lo dicono spesso.

"break the balls/nuts" something or someone really annoying. It's a bad word anyway

It's indeed pretty vulgar, but still a legit language question!

It literally means "to break the testicles". Coglioni is a bad word for testicles.
There is also the variant "rompere le palle"
Palle = balls
Same meaning, same usage.
You use this phrase when you're extremely annoyed at someone or if this someone has really pissed you off.
Example:
Non rompere i coglioni! = stop pissing me off!
